The 31-year-old received treatment and played on until he was ultimately replaced at half-time by Nacho Fernandez.
"He couldn't continue. We'll have to see how long he's out for," said boss Zinedine Zidane.
Ramos is shown with blood covering his hand, face and top, in a post shared with his 20.3 million followers on Instagram. The 26-year-old coupled the snap with a caption that read: "I would bleed a thousand times for this badge and this shirt.
Volvería a sangrar una y mil veces más por este escudo y esta camiseta. Gracias a todos por el cariño y apoyo. En nada, de vuelta. I would bleed a thousand times for this badge and this shirt. Thanks for your support. I'll be back in no time. #HalaMadrid

The European champions have not revealed a timeframe for the centre-back's return.
A club statement read: "He [Ramos] has been diagnosed with a fractured nose. His recovery will continue to be assessed."
Madrid, who are now 10 points behind league leaders Barcelona, travel to Apoel Nicosia for Tuesday's Champions League clash.
